PATIENT INFORMATION

The patient information section is designed as a resource for potential and current patients, providing you with a convenient location to find paperwork, educational information, and answers to commonly asked questions. Initial Visit

Your physical therapist will perform a detailed examination; that may include an assessment of your symptoms, strength, flexibility, balance, coordination, posture and body mechanics. Other thing he may assess or evaluate may include:

Joints, Muscles, and Soft Tissues
How you walk (“gait analysis”)
How you get up from a lying position or get in and out of a chair (“functional or transitional activities”)
How you use your body for certain activities, such as bending and lifting (“body mechanics”).

What to Bring/Expectations

Prescription (most cases unless PPO), even if doctor’s prescription is to another clinic. It is your choice where you go
Driver’s License/Valid ID
Insurance Card
If possible Print and fill out corresponding paperwork at home or come in 15 minutes early to fill our your paper work
Any imaging (x-rays, MRI, etc.) reports
Wear comfortable clothing you can move in
Expect to stay 1-1.5 hours

Accepted Insurances

Progressive Physical Therapy Plus accepts most major PPO plans, including Medicare, Medi-Medi, Workers’ Compensation, and TRICARE. If you don’t see your plan here, please feel free to give us a call. We also offer special pricing for cash patients, and a written estimate will be provided. We also offer a discounted fee for our cash-based patients.
